About the role

Provides direct care to residents while promoting their highest level of functioning as directed by management. Assists in maintaining a safe and clean environment. Ensures quality services within facility standards, State, and Federal regulations.

Responsibilities

  • Receive and give report to management on resident status at the beginning and end of each shift and as needed
  • Notify management of any changes in resident physical and/or mental status
  • Provide personal care to residents per individualized plan of care
  • Document all pertinent information regarding care
  • Prepare residents for meals, assist to and from dining room, and help with meal setup and eating
  • Absist residents with toilet and incontinence care
  • Make room checks and verify resident location as directed
  • Keep residents’ apartments neat and clean
  • Perform clinical procedures according to facility standards and scope of practice regulations
  • Perform vital signs, blood pressure, weights, and other trained procedures
  • Complete prevention and restorative care including ambulation, transferring, bathing, grooming, and all other Activities of Daily Living
  • Maintain knowledge of facility infection control procedures
  • Absist in turning, positioning, and repositioning residents as outlined in the service plan
  • Participate in Interdisciplinary Team Process including documentation of care and attending care plan meetings
  • Communicate effectively with residents, families, visitors, healthcare team members, and other disciplines
  • Actively participate in facility’s Quality Assurance Program
  • Report elder maltreatment or abuse to Director of Health and Wellness, Executive Director, and/or State
  • Maintain education, compliance with regulations, and current knowledge of resident care practices
  • Follow facility emergency procedures including fire and disaster drills
  • Maintain resident confidentiality and adhere to HIPAA policies
  • Perform other duties as assigned

Requirements

  • Certification as a CNA from an approved State course
  • Six months’ experience in long-term care preferred
  • Meet all health requirements, including TB testing
  • Pass state-required fingerprinting and criminal background check
  • Compassion for and desire to work with the elderly
  • Skilled in conflict resolution and active participation in resident care assessments
  • Demonstrates leadership, communication, and coaching skills
  • Strong written, verbal, and interpersonal skills
  • Excellent organizational skills, detail-oriented, and able to manage time effectively
  • Pleasant, professional, and personable manner
  • Ability to understand and communicate effectively with residents, guests, supervisors, co-workers, and vendors

Pay

Salary range: $15 - $18 per hour
