The Certified Nursing Assistant (CNA) works under the direction of the nursing leadership team to provide compassionate care to assigned hospice patients in accordance with their assessments and individualized care plans.

Responsibilities

• Collaborate with the hospice care team to meet patient needs, promote comfort and dignity, and report changes in condition.
• Follow all company policies, procedures, safety guidelines, and infection‑control practices.
• Provide direct, one‑on‑one patient care while being supported by a dedicated care team.
• Work independently and manage daily responsibilities.
• Travel to patient locations as needed, using reliable transportation.

Requirements

• Current CNA certification in the state(s) of practice.
• Valid driver’s license.
• Reliable transportation.
• Current auto insurance.
• Willingness to travel.

Qualifications

• Compassionate and committed to improving patients’ quality of life.
• Ability to work collaboratively with nurses, social workers, chaplains, volunteers, and aide leadership.
• Comfortable working one‑on‑one with patients while being supported by a dedicated care team.
• Able to work independently and manage daily responsibilities.
• Eagerness to pursue additional certifications, education, and career growth opportunities.
