As a CNA, you'll play a vital role in supporting our patients' quality of life through consistent, one-on-one, relationship-based care in their homes. Compensation $16.00-$22.00/hr depending on experience $1/hr shift differential for nights and weekends Offer mileage reimbursement Benefits PTO (Full-Time and Part-Time employees only) Medical, Dental, and Vision (Full-Time only) Employee Assistance Program (EAP) 401(k) Option Shifts Travel to Montrose, Delta, and Cedaredge Full-Time - 30 plus hours a week Part-Time - less than 30 hours per weekPRN - minimum of 12 hours every month Key Responsibilities Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility) Provide companionship, emotional support, and social engagement Support meal preparation, nutrition support and light housekeeping Monitor and report changes in condition Follow care plans and document accurately using EMR standards Qualifications High school diploma or GED Active CNA certification in Colorado CPR certified Valid driver's license and reliable transportation Ability to work independently in a non-facility setting Home health, long-term care, or hospice experience preferred.
