Duties
- Checks/documenting patients’ pulse, temperature and respiration.
- Affords assistance to patients in simple prescribed exercises and range of motion.
- Maintains records to report and discuss observations with a supervisor or case manager.
- Provides patients with help moving in and out of beds, baths, wheelchairs or automobiles, and with dressing and grooming.
- Changes bed linens, washes and irons patients' laundry, and cleans patients' quarters.
- Performs a variety of duties as requested by client, including running errands, preparing meals, etc.
- Functions as a team member, collaborating with and responsible to the patient, physician, and healthcare team.
- Communicates significant changes in patient status to the RN Case Manager.
- Attends Multi-Disciplinary Team Meetings and documents attendance.
- Communicates to RN Case Manager/Director of Clinical Services as needed.
- Participates in Agency's other mandated activities as requested by Agency Director.
Requirements
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education to reflect ability to perform job as described.
- Current, unencumbered state certification to practice as a Certified Nursing Assistant (CNA).
- Professional experience required per state regulations to perform the job as described.
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
- Provide copy of current TB results or Chest X-Ray.
- Provide copy of current CPR.
- Flexible, organized, and able to exercise sound judgment to perform job duties/responsibilities as described.
Physical Requirements
- Regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms.
- Frequent lifting and/or moving up to 15 pounds, and occasionally lifting or moving 100 pounds or more if applicable.
- Ability to perform the anticipated level of physical assist required to provide safe client care.
Work Environment
- The work environment is highly variable, as care is generally provided in private homes.
- Lighting, acoustics, air quality, temperature and environment hazards are not predictable.
- The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
- The employee is required to travel frequently to complete home health visits.
