Salary: $19-22/hr | Full-time, Monday–Friday, 8 AM–4:30 PM with on-call rotation weekends every 8–10 weeks | Toledo, Ohio and surrounding areas
Requirements
- Licensed Nursing Assistant in the state of practice (CNA)
- CPR/BLS Certified
- Prior experience working in an AL/LTC/SNF facility or Home Health (REQUIRED)
- Enjoy working with senior citizens; positive attitude, professional, responsible, and dependable
- Must be willing to travel
- Valid driver’s license, auto insurance coverage, and reliable transportation
- Hospice experience preferred
Responsibilities
- Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care
- Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner
- Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life
- Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments
- Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care
- Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts
- Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse
- Accurately document care provided in accordance with Care 360 Hospice policies and procedures
- Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences
- Provide support and education to family members and caregivers within the scope of the CNA role
- Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance
