About the role
As a Bluegrass Care Navigators' Hospice Team member, you will be part of a team providing care and support to patients and their families who no longer receive curative treatments for terminal illness but instead choose care to improve comfort and quality of life. Our service allows team members to focus on one patient at a time, providing care where the patient calls home—whether a house or apartment, nursing home, assisted living facility, or hospital. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, care can be provided at one of our Hospice Care Centers. Responsibilities
- Works under the supervision of a Registered Nurse (RN).
- Provides personal care and support services to meet the patient’s daily needs, enhance comfort, and maintain a safe environment.
- Observes the patient’s condition.
- Accurately reports and documents observations and the services provided.
Qualifications
- At least 18 years of age and a high school graduate or equivalent.
- Currently registered as a State Nurse Aide with the Kentucky Board of Nursing.
- Completion of a recognized Certified Nurse Assistant (CNA
