Position Highlights

  • Follows care plan developed by RN.
  • Reports client changes in condition to RN, as described on care plan.
  • Assists clients with household tasks including, planning, and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
  • Measures and records client's oral, rectal, and axillary temperatures, pulse respiration and blood pressure as ordered.
  • Performs range of motion and other simple procedures as an extension of therapy services as assigned.
  • Assists clients with use of equipment (side rails, foot stools, wheelchairs, etc.) to meet safety needs.
  • Performs or assists clients to perform activities in accordance with the personal care services plan, such as changing ostomy appliances or external catheter care.
  • Participates in multi-disciplinary case conferences as appropriate.
  • Accurately documents services provided in accordance with agency policy on day services are rendered.
  • Submits documentation in a timely manner.
  • Complies with agency policies and procedures.
  • Works with scheduling to assure accurate and up to date schedule.

Schedule

Monday - Friday, 8am - 5pm, 40 hours a week. No weekends or holidays.

Minimum Qualifications

  • High School diploma or GED required.
  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
  • Licensed CNA within the State of Michigan
  • Must have current Driver's license and reliable transportation to and from work site.
  • Documentation of one year's employment as an aide in an institution, home care, or hospice setting.
  • Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
  • Must accept responsibility for maintaining skills, learning, and adhering to agencies policies and procedures on an on-going basis.
  • Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics and adhere to the Compliance Program.
