Responsibilities

  • Deliver and assist residents with daily needs, including personal care, preparation for activities or social programs, and routine activities in accordance with accepted standards of practice, state and federal regulations, and licensing requirements.
  • Provide exceptional nursing care to residents within the scope of CNA responsibilities and complete nursing procedures under the direction of the Licensed Nurse Supervisor/Charge Nurse.
  • Administrative:
    • Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
    • Report all changes in resident’s condition to the Charge Nurse/Nurse Supervisor immediately.
  • Nursing Care:
    • Weigh residents in accordance with their plan of care and report significant weight loss or gain to the Charge Nurse/Nurse Supervisor.
    • Measure residents’ temperature, pulse, respirations, blood pressure, weight, intake, output, and food consumption; document in the official medical record.
    • Keep residents’ water pitchers clean, filled with fresh water on each shift, and within easy reach of the resident.
    • Prepare residents for meals (e.g., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, escort to/from dining room).
    • Record the resident’s food/fluid intake and report changes in eating habits.
    • Assist residents with activities of daily living (ADLs), ranging from minimal assistance to total dependent care.
    • Contribute to the development and implementation of interventions based on resident needs, goals, and care preferences.
    • Monitor and evaluate the resident’s response to care plan interventions and treatments in accordance with facility policies.
    • Care for residents with mental and psychosocial disorders, including those with a history of trauma or post-traumatic stress disorder.
    • Assist in preparing residents for exams, tests, and procedures; provide support or accompany residents to appointments.
  • Care Plan and Assessment:
    • Participate in the care planning process based on resident preferences and person-centered care.
    • Review care plans daily to determine if changes in the resident’s daily care routine have been made.
  • Admissions | Transfers | Discharges:
    • Assist nurses with admission, transfer, and discharge functions.
    • Greet residents, escort them to their room, and provide new resident orientation to the unit/facility.
    • Introduce residents to their roommate (if any), other residents, and personnel.
    • Inventory and mark residents' personal possessions as instructed; update and revise the personal inventory list.
  • Staff Development:
    • Attend and participate in facility in-service training programs, including resident rights, prevention of abuse and neglect, dementia care, behavioral management, and CNA competencies.
    • Attend scheduled orientation programs and activities.
    • Participate in scheduled training and educational classes to maintain current CNA certification.
  • Safety | Sanitation:
    • Store all tools, equipment, and supplies before leaving work area for breaks and at the end of the workday.
    • Clean, disinfect, and return all resident care equipment to its designated storage area after each use.
    • Follow established infection prevention and control procedures.
    • Follow established procedures in the use and disposal of personal protective equipment (PPE).
    • Keep the nurses’ call system within easy reach of the resident.
    • Conduct routine housekeeping duties (e.g., clean bedrails, overbed table, nightstand) related to nursing care procedures.
    • Report all hazardous conditions and equipment to the Charge Nurse/Nurse Supervisor immediately.
    • Report all safety violations.
    • Report any communicable or infectious disease to the Director of Nursing and/or Infection Preventionist.
    • Report missing/illegible labels and safety data sheets (SDSs) to your supervisor.
    • Clean and disinfect multi-resident use devices and equipment (wheelchairs, walkers, etc.) as instructed.
  • Equipment | Supplies:
    • Report defective equipment to the Charge Nurse/Nurse Supervisor; utilize lockout/tagout as directed.

Requirements

  • Texas Licensed Certified Nursing Aide (CNA) certification (required).
  • High School Diploma or Equivalent (preferred).
  • Demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of residents served.
  • Ability to make independent decisions when circumstances warrant such action.
  • Tactful communication with personnel, residents, family members, visitors, government agencies, and the general public.
  • Willingness to work harmoniously with other personnel.
  • Patience, tact, cheerful disposition, enthusiasm, and soft skills to handle difficult residents.
  • Openness to adopting new methods and principles into existing nursing practices.
  • Ability to relay information concerning a resident’s condition.
  • Must not pose a direct threat to the health and safety of others in the workplace.
  • One year of experience in a Skilled Nursing Facility environment (preferred).

Physical and Sensory Requirements

  • Able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Schedule

Twelve-hour rotating shift: Night, 6:00 PM to 6:00 AM.
