About the role

Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Responsibilities

• Provides personal hygiene care to hospice patients following the patient's Plan of Care
• Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing
• Deliver routine medical supplies as needed to patients
• Observe and report any changes in the patient's condition to the Nurse/Care Manager
• Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT, and provide excellent customer service
• Successfully complete 12 hours of in-service training annually
• May assist the patient with transfers as needed
• Provide compassionate companionship to patients while delivering services

Requirements

• HS Degree or GED required
• Proof of State Certification - Certified Nurse Aide required
• 1 year of direct patient care experience required
• Must demonstrate skills and written competency before working independently with the patient
