Certified Home Health Aide (CHHA) Hospice

Job Description

Spanish Cove at Home hospice is a new agency looking for a Certified Home Health Aide (CHHA) to join their team. The position offers the chance to make a difference in the lives of those they care for, in a supportive environment.

Essential Job Functions and Responsibilities

• Follows written instructions for patient care as prepared by an RN.
• Assists patients with personal care (including lifting and transferring patients) to maintain good hygiene and a healthy environment.
• Performs simple procedures as an extension of therapy services, such as ambulation, ROM, and exercise.
• Administers medications as directed by a physician or hospice nurse, following state regulations.
• Reports any changes in the patient's condition or needs to the supervisor or RN.
• Maintains records of services provided and the patient's condition.
• Maintains a positive relationship with the patient and family members.
• Participates in patient care conferences to contribute to the overall care plan.
• Performs necessary household tasks to support the health care environment at home.
• Manages time effectively.

Qualifications

• Must be CNA-LTC and Home Health Aide (HHA) certified.
• Current CPR certification required.
• Frequent lifting and moving capabilities, including the ability to lift, move, and transfer up to 100 lbs.
• Valid driver’s license and reliable transportation with appropriate insurance coverage.
• Willingness to adhere to health and safety standards.
• Supportive and compassionate attitude.

Education and Experience

• A high school diploma or GED is required.
• One year of relevant experience is preferred.
• Must complete a training and competency evaluation program or equivalent approved by the state, with no more than a 24-month gap.
