About the role
This Certified Home Health Aide will be responsible for, but not limited to:
- Providing or assisting in activities of daily living: personal hygiene, grooming, mouth, skin care and nails
- Providing assistance in toileting; positioning and transfer activities; frequent turning and positioning of a bed bound resident/patient
- Assisting with prescribed exercise and range-of-motion activities as taught by the Hospice Nurse
- Assisting with light housekeeping of the individual’s personal unit or room, including changing bed linens and patient laundry
- Feeding and assisting with meals
- Obtaining temperature, pulse and respirations as supervised by the Hospice Nurse
- Reporting any changes in the resident’s/patient’s condition to the Hospice Nurse (and to the facility nurse if in a facility), documenting the information on the Home Health Aide Certification of House form with date, time and recipient
- Assisting or reminding the patient to take medication in compliance with applicable regulations
Requirements
- High School Diploma or equivalent
- State Certified CHHA license
- Must be willing to travel
- Valid driver’s license and reliable transportation
Preferred qualifications
- Experience in hospice or home health
- 1 year of relevant experience
