About the role
Swan Hospice provides quality hospice care to patients in skilled nursing facilities, assisted living facilities, and private homes. Responsibilities
- Assists with prescribed exercise and range-of-motion (ROM) activities as taught by the Hospice Nurse.
- Provides assistance in toileting, positioning, transfer activities, and frequent turning of bed-bound residents/patients.
- Assists with activities of daily living (ADLs), including personal hygiene, grooming, mouth care, skin care, and nail care.
- Assists or reminds the patient to take medication in compliance with regulatory and accrediting bodies.
- Performs light housekeeping of the patient’s personal unit or room, including changing bed linens and handling patient laundry if needed.
- Feeds and assists with meals.
- Obtains temperature, pulse, and respiration as supervised by the Hospice Nurse.
- Reports any changes in the resident’s/patient’s condition to the Hospice Nurse and, if in a facility, to the facility nurse. Documents the information, including the date, time, and person receiving the report.
Requirements
- HHA License in the state of New Jersey.
- Understands the hospice philosophy.
- Minimum of one year of experience working in hospice.
- Dependable transportation.
