Certified Diabetes Care and Education Specialist (CDCES)

Role Responsibilities:

• Serve as the Diabetes Self Management Education (DSME) Program Coordinator for the facility
• Oversee the planning, implementation and evaluation of the Diabetes Self-Management Program
• Absess the needs of each patient with diabetes and plan the care needed to meet those needs, including the spiritual, emotional and physiological needs
• See that the plan of care is carried out and coordinates the care of the patient by communicating with other members of the care team
• Provide diabetic education and support to patients and families in both the inpatient and outpatient settings
• Complete nutritional assessments and provide nutritional counseling to all UHH patients as needed
• Participate and attend UHH meetings as needed
• Design and participate in community nutrition programs as needed
• Work with physicians, nursing staff and other ancillary staff professionals to provide high quality care for patients
• Maintain a thorough knowledge of and utilize current clinical practices of medical nutrition therapy for diabetic patients
• Develop appropriate educational materials for patients served
• Participate in menu planning for patients
• Function as part of the health care team using interdisciplinary approaches in delivering care to patients/residents

Qualifications:

• Candidates must have a bachelor or advanced degree as a dietitian, registered nurse, pharmacist, physical therapist or occupational therapist from an accredited school
• Completion of ADA approved dietetics internship or equivalent education experience
• Be licensed as a Registered Dietitian, Registered Nurse, Pharmacist, Physical Therapist or Occupational Therapist in the State of Wisconsin
• Be a Registered Dietitian with the Academy of Nutrition and Dietetics if applying for the position as a dietitian
• Candidates must be licensed as a Certified Diabetes Care Education Specialist (CDCES) or be CDCES eligible
• Knowledgeable of patient care standards according to the State of Wisconsin and in accordance with the Academy of Nutrition and Dietetics and American Association of Diabetes Educators (AADE)
• Have academic preparation and/or experience in the care of persons with chronic disease
• Previous experience using the EPIC medical record system is preferred but not required
• Previous experience with the Microsoft Office Suite software programs is beneficial

• This position is eligible for “up to a $5,000.00 Employment Bonus” which is prorated based on the FTE. Bonus payments will be made based on the FTE schedule below:
• 1.0 FTE $5,000
• 9 FTE $4,500
• 8 FTE $4,000
• 7 FTE $3,500
• 6 FTE $3,000
• 5 FTE $2,500
