Responsibilities

• Provide personalized care and support to patients in their homes, promoting their comfort, dignity, and independence
• Assist with daily living activities, such as bathing, dressing, and grooming
• Monitor and report changes in patients' condition to healthcare professionals
• Collaborate with healthcare teams to develop and implement individualized care plans
• Maintain accurate and detailed records of patient care and progress
• Demonstrate a commitment to patient-centered care and a passion for improving the lives of others

Requirements

• Certified Caregiver certification (or willingness to obtain within a reasonable timeframe)
• High school diploma or equivalent required
• Ability to work varying shifts, including weekends and holidays
• Strong communication and interpersonal skills
• Ability to maintain confidentiality and adhere to HIPAA guidelines
• Compassionate and empathetic approach to patient care
