Responsibilities
- Provide care for patients with acute and chronic illnesses in older adults
- Take accountability for patient care outcomes and seek specialist consultations as needed
- Engage with hospitalists regarding patient admissions
- Lead care team for patients able to visit the office
- Cookordinate with transitional care teams for homebound or hospitalized patients
- Facilitate weekly Super Huddles and Transforming Care Meetings
- Manage, mentor, and coach PCPs to achieve clinical goals
Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
- Current, active MD licensure in State of employment
- Minimum of 1-year clinical experience in related settings preferred
- Board certification in relevant specialty preferred, Board Eligibility required
- Basic Life Support (BLS) certification from American Heart Association or Red Cross required within first 90 days of employment
Pay
$238,832 – $341,189 annually
