About the role

Provides ongoing case management services for children and youth with special health care needs and their families within a medical home model of care.

Responsibilities

• Coordinates appointments for specialists and schedules medical tests, exams, and assessments for patients.
• Assists families with financial and social assistance.
• Ensures participation in the continuous planning of patient care.

Requirements

• Bachelor’s degree in Social Work (essential).
• One year of relevant experience (essential).
• Basic Life Support certification (essential).
• AHCA Level II certification (required).
