CDS Nurse Case Manager

About the role

The Marklund is seeking a dedicated Nurse Case Manager for its Community Day Service program. The position is based in Geneva and offers a salary range of $82,000 - $87,000 based on qualifications and experience.

Responsibilities

• Serve as the Administrative and Clinical Coordinator of all nursing activities for the day program.
• Administer medications, tube feedings and treatments as prescribed.
• Supervise nursing and direct care staff.
• Work with staff, parents and regulatory bodies to ensure all appropriate measures are taken in relation to resident care.
• Plan, develop and coordinate Quality Improvement Programs to enhance the quality of resident care.
• Monitor Infection Control Program, in accordance with policies and procedures, to ensure the highest degree of quality resident care and safety can be maintained at all times.

Requirements

• Valid nursing license in the state of Illinois, current on the Illinois Health Care Worker Registry.
• Possess current BLS CPR certification for Healthcare Workers.
• Minimum of one year of nursing experience.
• Able to perform the essential functions of the job and to meet its functional and environmental requirements.
