Description

The Clinical Documentation Integrity (CDI) Consultant at e4health will provide timely reviews of patient medical records to ensure accurate and complete documentation reflecting the patient's severity and complexity of illness. This role involves working with providers and coders to ensure documentation accurately reflects the patient's condition.

  • Work with providers and coders to ensure documentation on the chart reflects the complexity of the patient.
  • Educate and interact with providers to improve documentation accuracy.
  • Conduct regular audits using standardized methodology to ensure compliance with Present on Admission (POA), Hospital-Acquired Condition (HAC), and Patient Safety Indicator (PSI) documentation.
  • Track and trend data for use in quarterly data quality reviews by the HIM Documentation Quality and Education Manager.
  • Escalate any observed issues impacting documentation or coding quality.
  • Affirm coding and DRG assignment compliance with CMS rules and regulations, as well as current coding practices and conventions.
  • Apply information related to coding changes and DRG and quality score changes affecting data quality and/or revenue via Case Mix Index (CMI).

Physical Demands

Sitting, talking, hearing, and near vision are required over 90% of the time. Feeling and reaching are required over 90% and 50% of the time, respectively. Ability to travel to field sites may be required up to 15% of the time.

Working Conditions

Over 90% of the time is spent indoors, with protection from weather conditions. Noise levels may be distracting or uncomfortable in unusual situations.
