CCHIP SEXUAL HEALTH CLINIC COORDINATOR

Location: Ruth M. Rothstein CORE Center, Department: CORE Services. Shift: 8:00 AM to 4:00 PM.

About the role

The CCHIP Sexual Health Clinic Coordinator will provide patient-centered HIV counseling, testing, PrEP education, assessments, and referrals for initiation to adults and young adults (adolescents) in the Sexual Health Clinic, community settings, and any Cook County Health (CCH) clinic where sexual health and PrEP services are provided, under the direction of the Manager of Patient Centered Care. This position develops, implements, and provides patient education on related topics (e.g., PrEP, HIV, STIs, hepatitis C, medication adherence). The role maintains a linkage database, submits monthly reports of program activities, and participates in administrative and staff development meetings. This is a grant-funded position under the 2026 CCH Ryan White Part C Outpatient grant, set to expire on July 31, 2026, with potential renewal.

Responsibilities

• Provides patient-centered HIV counseling, testing, HIV/STI/PrEP/DoxyPEP education, assessments, and referrals for adults and young adults in the Sexual Health Clinic, community settings, and CCH clinics.
• Delivers patient education on HIV transmission, adherence, treatment, and related health topics (e.g., hepatitis, dietary needs, risk reduction, diabetes, hypertension) in collaboration with the Manager.
• Coordinates and monitors PrEP-related care and services.
• Supervises health education staff providing sexual health services.
• Assesses individual and group health education needs in the Sexual Health Clinic and community settings.
• Maintains current knowledge of HIV and other STIs, including etiology, transmission, treatment, PrEP, risk reduction, nutrition, diabetes, hypertension, hepatitis, and Partner Services (PS).
• Links patients to HIV/HCV care, PrEP services, medication management, adherence, and treatment; conducts outreach/home visits as needed.
• Assists with linking HIV-positive patients on the Stroger campus, Emergency Department, and other CCHIP sites to patient-centered model homes (PCMH).
• Provides post-test HIV counseling in the Stroger ED (adult and pediatric) and other CCHIP sites as referred by providers and at community testing events.
• Conducts community outreach for prevention, treatment, and linkage to care.
• Collaborates with nursing staff to manage Sexual Health clinic operations.
• Documents all face-to-face interactions, phone calls, and collateral contacts in the EMR (Electronic Medical Record) and databases (CAREWare/PROVIDE).
• Updates and maintains policies and procedures for the Sexual Health clinic.
• Completes departmental reporting, including PrEP monthly reports (CDOH) and Prevention & Education reports.
• Assists with development of peer educators and integrated care specialists.
• Reviews grant-related budgets.
• Co-facilitates monthly sexual health meetings.
• Performs other duties as assigned.

Requirements

• Bachelor’s degree from an accredited college or university (official transcripts required at time of interview).
• Two (2) years of HIV counseling experience.
• One (1) year of experience supervising or managing staff.
• HIV Testing and Counseling certification (certificate must be presented at time of interview).
• Proficiency in Microsoft Office (Excel, Word, PowerPoint).
• Valid driver’s license and insured vehicle or reliable transportation for moderate local travel.
• Flexibility to work evening and/or weekend hours.

Preferred Qualifications

• Bachelor’s degree or higher in a health-related field, education, psychology, or social science.
• Partner Services experience and certification.
• Bilingual proficiency.

Physical and Environmental Demands

This position operates within a healthcare environment. The incumbent must adhere to hospital and department-specific safety requirements, including Personal Protective Equipment (PPE), hand hygiene, and departmental engineering/work practice controls. Compliance with all safety policies and procedures is mandatory.

Pay

Pay range: $61,730–$64,850 yearly. Final salary and offer components are subject to approval based on Cook County Health (CCH) policy. Placement within the range depends on factors such as work experience and internal equity. For union-represented positions, placement follows collective bargaining agreement rules.
