Responsibilities
- Performs comprehensive, in-depth assessment and clinical formulation, development of accurate diagnosis and strengths-based treatment plans for CCBHC services.
- Provides individual/group therapy, crisis intervention, and case management services, adhering to CCBHC and MRO/Medicaid rehabilitation service models.
- Provides a full range of therapeutic interventions, advocacy, skill-building, and related services.
- Learns and utilizes agency and community resources.
- Assesses community conditions that affect clients to identify resource gaps and problems.
- Works with multidisciplinary professionals including psychiatrists, nurses and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
- Provides on-call or in-person crisis intervention services when necessary.
- Recommends financial assistance and other benefits the client may be entitled to.
- Serves on regional or agency-wide committees.
- Maintains timely and accurate client records and any necessary agency and CCBHC required reporting.
- Participates in agency and site based required meetings and trainings.
- Works effectively with diverse staff and service population.
Qualifications
- Master's Degree in Social Work, Counseling or related field from an accredited college or university, required.
- Licensed Social Worker (LSW), or Licensed Professional Counselor (LPC), required.
- Must obtain and maintain IM+CANS certification, required.
- 2+ years of post-graduate social work experience at an intermediate skill level, preferred.
- Certified Alcohol and Drug Counselor (CADC) or experience working with clients experiencing substance use preferred.
- Licensed Clinical Social Worker (LCSW), preferred.
- IM+CANS certification, preferred.
- ASAM certified, preferred.
Additional Requirements
- Satisfactorily pass all relevant screenings and criminal background checks.
- Driving for work required with a personally owned vehicle.
Physical Demands
- Sit and stand for sustained periods of time.
- Frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls.
- Occasionally stand, bend, stretch, stoop, kneel and crouch.
- Close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
- Occasionally lift and/or move up to 25 pounds.
Pay
The average starting salary for this position will fall in the range of $59,000-$70,000 annually. $2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment).
