About the Role

The Registered Nurse at Apalachee Center, Inc. provides case management and nursing services to coordinate all aspects of care for individuals with complex medical and psychiatric needs. The primary goal is to optimize client functioning by ensuring quality treatment and support services are delivered efficiently and effectively.

Responsibilities

• Assessment: Conduct comprehensive nursing assessments considering mental and physical health. Screen and address medical problems, risk factors, and obstacles to care. Update assessments per Client Record Handbook guidelines and finalize documents in Avatar within 48 hours.
• Treatment Plan: Develop individualized service plans with measurable goals in partnership with clients and families. Assist with implementation, emphasizing family autonomy. Update plans every 3 months or as client conditions change.
• Nursing Services: Implement psycho-pharmacologic and medical treatments, including medication administration and management. Organize medication distribution and provide individual, group, and family support in office and community settings.
• Patient Care: Perform patient assessments, monitor for medical issues and medication side effects, plan care, provide crisis intervention, advocate for clients, make referrals, complete documentation, and engage in health promotion and quality improvement activities.
• Progress Notes: Document all services in the client record per guidelines. Finalize progress notes in Avatar within 48 hours, ensuring they reflect efforts linked to service plan goals and client progress.

Qualifications

Registered Nurse (RN) license in the state of Florida.
