About the role
Work is performed in accordance with established regulations, policies and procedures and requires initiative and independent professional judgement in the course of duties. The Director of Social Work reviews work through conferences and evaluations.
Responsibilities
- Conducts and coordinates admissions process to provide continuity of care to new residents, as assigned.
- Counsels residents and members of their family to establish social, economic, emotional and health problems and determines the kind of service needed on the basis of established policies.
- Aids residents and family members to achieve a satisfactory adjustment to their new surroundings.
- Performs in an advisory capacity to other professional staff in specialized areas through participation in conferences and meetings for diagnosis and treatment planning.
- Works in close cooperation with other community services in planning to meet the needs of residents and assists the residents through the utilization of these resources.
- Interprets and explains programs, policies and procedures to the residents and their families.
- Prepares social studies, case histories; writes letters; makes reports; and keeps records.
- Acts as Discharge Coordinator for initiating and coordinating discharge planning in accordance with established procedures.
- May be required to perform additional work that follows prescribed or well-established procedures that can be learned within a reasonable time by training on the job.
- Performs similar or related duties as assigned or required.
Requirements
- Bachelor's Degree with a Social Work major; or
- One-year experience in public or private social work and a Bachelor's Degree; or
- Any equivalent combination of education, experience, and training.
