Caseworker Home Care

About the Role

The Caseworker is a key member of patient care operations, primarily responsible for assisting in the provision of direct casework services to patients and their families within the hospice program of care.

Qualifications

• Minimum of two (2) years of experience in a healthcare environment with a primary focus on community resources utilization and patient contact.
• Reliable transportation with adequate insurance coverage for driver and passenger, or the ability to manage the stress of repeated loss.
• Extensive knowledge of community resources within the specific hospice community.
• Knowledge of local, county, state, and federal assistance programs.
• Ability to work with minimal supervision.
• Proficiency or ability to become proficient in company software programs.

Education

Bachelor's Degree in Social Work or a related Social Science field from an accredited school.
