Caseworker -Empowerment Program (FT $21.00/Hr)

Responsibilities

• Engage in client interviews to assess and understand the nature and extent of their challenges.
• Gather relevant information about the client's medical, psychological, and social factors.
• Evaluate the client's capacities and provide counseling and support to develop plans for meeting their needs and improving their social functioning.
• Actively refer clients to community resources and organizations.
• Maintain accurate documentation and reporting for government agencies and funding sources.
• Ensure effective communication with supervisors and other stakeholders.
• Timely reporting of safety issues is an essential responsibility.

Qualifications

• Education and/or Experience Required at Entry: Associate Degree in Human Services or a related field.
• Three years of experience working with vulnerable individuals.
• Alternatively, a combination of education and relevant experience will be considered.
• Candidates with lived experience are preferred and encouraged to apply.
