About the role
Under the general direction of a Case Management Supervisor, responsible for the full range of case management duties including cases requiring intensive case management and high levels of expertise.
Responsibilities
- Implements the clients' service plans.
- If assigned to support young children (ages 3-6), will be responsible for assisting Intake with the process of determining Lanterman eligibility for children under “Provisional Eligibility” criteria prior to turning 5 years old.
Requirements
- A Master's degree in a field related to developmental disabilities.
- OR
- A Bachelor's degree in a field that offers content related to developmental disabilities, such as social work, psychology, counseling, special education, or nursing, and three years of employment in a field related to developmental disabilities.
Qualifications
- The minimum education requirement may not be waived.
Skills
- Comprehensive knowledge of developmental disabilities.
- Strong interpersonal and communication skills.
- Ability to work independently and manage multiple tasks.

Pay
$62,244 - $65,388 per year.
Schedule
8-hour shift. Day shift. Monday to Friday.
