Case Manager-SW Afternoons

About the Role

The Case Manager – Social Worker provides therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to facilitate continuity of care in the community. Demonstrates positive interpersonal communication with patients, families, visitors, and co-workers in accordance with organization and departmental policies and procedures.

Responsibilities

• Provide therapeutic intervention and social work services to patients and families.
• Ensure comprehensive, integrated, and uninterrupted care in the hospital and community.
• Apply knowledge of social work practices, case recording, chart documentation, and community referrals.
• Coordinate services for patients outside of the hospital using effective networking techniques.
• Understand and apply knowledge of how social, familial, economic, and individual factors affect patient functioning.
• Demonstrate knowledge of child and adult protection laws, state Medicaid guidelines, NASW code of ethics, and confidentiality standards.
• Assess and address patient needs relative to population-specific and age-specific requirements.
• Make independent clinical and psycho-social decisions regarding patient care.
• Provide input on the organization and functioning of the hospital unit.
• Conduct counseling, supervision of students, and consultation with colleagues.
• Document patient counseling sessions accurately.
• Collaborate with healthcare teams and interdisciplinary hospital teams.
• Understand CMS, commercial payer requirements, and hospital financial/reimbursement processes.

Requirements

• Master’s degree in Social Work required.
• Three (3) years of clinical experience.
• Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
• Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association preferred.

Schedule

Evening shift.
