Case Manager Social Worker - LMSW

About the role

Provide comprehensive case management services to patients in Carroll Hospital’s care management program. Collaborate with healthcare providers to develop and implement care plans that address physical, emotional, and social needs.

Responsibilities

• Conduct assessments to identify patient needs and develop personalized care plans
• Coordinate care among various healthcare providers and community resources
• Monitor patient progress and adjust care plans as necessary
• Communicate regularly with patients and their families to ensure they understand their health status and treatment options
• Ensure compliance with regulatory standards and hospital policies

Requirements

• Licensed Master of Social Work (LMSW)
• At least 2 years of experience in case management or related field
• Bilingual in English and Spanish preferred

Qualifications

• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite
