Essential Functions and Responsibilities
- Captures care of patients with clinical partners; assists patients in achieving realistic and desirable outcomes.
- Serves as a subject matter expert and provides guidance to care team members.
- Mentors new social work Associates and social work student interns in the Care Coordination department.
- Assesses long-term and future patient care needs by identifying potential changes in independence or functional quality.
- Updates clinical partners on activity statuses and treatment plans.
- Shares information and educates patients and their families about community resources.
- Develops, coordinates, and communicates discharge plans for medically complex and challenging social issues related to hospital progression.
- Documents assessments and overall discharge plans in the medical record.
- Aids with audits and assessments of discharge planning documentation, and addresses complex care coordination issues.
- Works with leadership to resolve delays in discharge, barriers to discharge, and identified trends.
- Handles immediate post-discharge patient needs and serves as a resource for intervention in cases of child/elder abuse/neglect and guardianship issues.
- Provides support for treatment decisions surrounding end of life and Medical Power of Attorney.
- Facilitates meetings and comprehensive care planning with interdisciplinary teams.
- Delegates tasks to support team members.
- Collaborates with contracted partners to facilitate post-acute facility placement.
- Conducts psychosocial assessments as needed for developing appropriate discharge plans for medically complex and challenging social issues.
- Participates in planning and implementing quality improvement activities.
Qualifications
- Master’s degree in Social Work required.
- Minimum of one year experience in social work required or two-semester internship completed within MWHC Care Coordination department.
- Experience with computer technology, specifically experience with Windows based programs, e-mail, and Microsoft Word required.
- Experience in a healthcare field preferred.
- Case Management Certification strongly desired.
Required Physical Requirements
- Constant (67-100%) use of arms and hands;
- Frequent (34-66%) standing, walking, and sitting;
- Occasional (0-33%) bending, stooping, and squatting;
- Ability to lift 35 lbs.;
- Ability to push and pull up to 20 lbs.;
- Auditory and visual skills to include color determination.
