Job Summary
This PRN position will be for weekday coverage at Methodist Hospital Texsan, part of HCA Healthcare.
Qualifications
- Graduate of an accredited nursing program
- RN license to practice in Texas or Active Multi-State RN Compact License
- Minimum of 3 years RN experience
- Prior experience in utilization review, resource management, discharge planning, or case management preferred
Responsibilities
- Support patients and families through care journey
- Coordinate services and connect resources
- Develop and implement individualized care plans
- Complete psychosocial assessments and discharge planning evaluations
- Collaborate with interdisciplinary team for seamless care
- Promote HCA Healthcare’s values of compassion, respect, and excellence
