Case Manager RN Per Diem - New Jersey
About the role
The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Responsibilities
- Advocates for the patient and family throughout the entire episode of care.
- Maintains ongoing communication with patients, families, and interdisciplinary team members to ensure effective care coordination.
- Makes rounds to assess patient needs and collaborates with healthcare providers to develop and implement individualized care plans.
- Maintains accurate documentation of patient care activities and outcomes.
- Monitors patient’s progression, intervening as needed to ensure efficient patient progress.
- Coordinates all aspects of discharge planning and establishes a safe discharge plan.
- Ensures compliance with regulatory requirements and hospital policies.
Requirements
- M-F 8:430 1 weekend day shift every 4 weeks 1 major holiday
- Willingness to occasionally float to all 3 campuses
- Required BSN
- 2 years of medical surgical nursing experience
- PREFERRED: 1-2 years of acute care, homecare or population health RN case management experience
- Certified Case Manager (CCM)
Qualifications
- Preferred CCM certification

- Strong communication and interpersonal skills
- Ability to prioritize tasks and manage multiple responsibilities
- Knowledge of healthcare regulations and policies
- Proficiency in electronic health record systems
Benefits
The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
