Case Manager, RN-Part-Time, East Campus

About the Role

A registered professional nurse at Erlanger Medical Center assists patients, families, physicians, and staff in managing the present healthcare plan. This role involves interacting with interdisciplinary teams to complete the continuum of care, participating in data gathering/reporting for JCAHO/DNV Core Measures, and managing the timely and appropriate use of resources.

The Case Manager arranges suitable aftercare to ensure efficient use of hospital resources and meets patient continuing care needs. They collaboratively assess, plan, coordinate, monitor, and evaluate patient treatment plans to assist in quality outcomes while maintaining adherence to departmental standards.

Responsibilities

• Assist patients, families, physicians, and staff in managing healthcare plans.
• Interact with interdisciplinary staff to complete the continuum of care.
• Participate in gathering and reporting data for JCAHO/DNV Core Measures.
• Manage timely and appropriate use of hospital resources through ongoing review.
• Arrange suitable aftercare to ensure efficient resource use and meet patient needs.
• Collaboratively assess, plan, coordinate, monitor, and evaluate patient treatment plans.
• Maintain adherence to departmental standards and organizational objectives.

Requirements

• Graduate from an accredited school of Nursing (required).
• Case Management certification with the ACMA within 2 years of hire (required).
• Current licensure to practice nursing in the State of Tennessee (required).
• For non-Tennessee residents, Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire.

Qualifications

• Broad clinical experience with a minimum of one year of experience and evidence of clinical knowledge (required).
• BSN (preferred).
• Case Management certification with ACM or CCM (preferred).
• Experience in utilization management or case management (preferred).
• Knowledge of managed care practices, community resources, regulatory and insurance reimbursement issues (preferred).

This is a part-time position located at Erlanger East, Chattanooga, TN.
