About the role
Emergency Department Utilization Care Managers perform utilization management functions to ensure medical necessity and the appropriate level of care. They assess, plan, implement, coordinate, monitor, and regulate compliance options and services to meet an individual's health needs throughout the continuum. They collaborate through communication and available resources to promote quality cost-effective outcomes. They facilitate concurrent documentation in the electronic medical record to accurately reflect care, services, and resources utilized in the delivery of patient care. They successfully meet requirements for productivity and accuracy related to all utilization management processes.
Responsibilities
- Assesses, plans, implements, coordinates, monitors, and regulates compliance options and services to meet an individual's health needs throughout the continuum.
- Collaborates through communication and available resources to promote quality cost-effective outcomes.
- Expedites linking patients with systems in the community in conjunction with ED MSW to provide them with resources, services, and opportunities to achieve and maintain maximum functioning.
- Facilitates concurrent documentation in the electronic medical record to accurately reflect care, services, and resources utilized in the delivery of patient care.
- Successfully meets requirements for productivity and accuracy related to all utilization management processes.
Requirements
- Minimum of 3 years of clinical experience.
- Knowledge of clinical and social factors that affect patient's functional status at discharge.
- Knowledge of community resources.
- Assessment skills.
- Florida Registered Nurse License.
- ACM (Accredited Case Manager), NACCM (National Academy of Certified Care Managers), CCM (Certified Case Manager), or CMCN certification preferred.
Pay
$31.78 - $42.90 / hour
