About the role
The RN Case Manager (CM) will assess patients for necessary services, provide resources, and coordinate care across various settings.
Responsibilities
- Facilitates coordination of care for ambulatory services such as infusion center, wound care, DME, and PT/OT.
- Facilitates discharge planning with the health care team for patients transitioning to the next level of care.
- Communicates with the health care team and patients/families regarding discharge needs and treatment options.
- Initiates hospital issued notice of noncompliance letters if necessary.
- Keeps supervisor informed of quality and risk issues.
- Identifies areas for process improvement and participates in related activities.
- Delegates tasks and duties to health care team members.
- Participates in on-call responsibilities and reports to the hospital within 30-45 minutes if needed.
Requirements
- Bachelor's Degree in Nursing (BSN).
- 2 years of acute care, broad-based nursing experience in a hospital setting.
- Preferred experience in quality management, discharge planning, or case management.
- Experience conducting clinical assessments, setting up home care services, and general discharge planning.
- Ability to communicate effectively and prioritize patient-related activities.
- Knowledge of the Kansas State Nurse Practice Act.
Qualifications
- Registered Nurse (RN) with a Kansas State license.
- Multistate license with the ability to practice within the State of Kansas.
