Case Manager RN

About the role

The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care.

Responsibilities

• Serves as a resource for discharge planning and care coordination for medically complex cases.
• Maintains current knowledge of available community resources/post-acute care options.
• Collaborates with other departments or disciplines to achieve desired quality outcomes.
• Makes sure quality of core processes in department.
• Applies the principles of safety and infection control to ensure the safety of all patients.
• Demonstrates superior interpersonal and communication skills.
• Works within a team to achieve patient and team goals.
• Shares and initiates regular and professional communication with co-workers.
• Participates in regular staff meetings.
• Identifies opportunities to improve processes based on feedback from staff and customers.
• Provides primary health care and performs selective medical services under the direction of practice physicians.
• Obtains complete history and physical data on patients.
• Reviews patient records.
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
• Establishes presumptive diagnosis.
• Orders appropriate laboratory and diagnostic procedures.
• Performs independent evaluations and treatment procedures in emergency situations.
• Provides monitoring and continuity of care between physician visits.

Qualifications

• Education: Associate's Degree in Nursing required. 4 Year/Bachelor's Degree, BSN, preferred.
• Experience: 2 or more years of healthcare experience preferred. Case Management or related experience preferred.
• Licensure: Current RN licensure in state of employment required.
• Additional Skills/Requirements: Weekend and holiday coverage. Additional Skills/Requirements Preferred: Knowledge/Competence of Microsoft Office products and keyboarding. Competencies: Patient Care, Quality, Communication, Medical Knowledge.

Physical Demands

The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
