Case Manager RN
About the role
The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care.
Responsibilities
- Serves as a resource for discharge planning and care coordination for medically complex cases.
- Maintains current knowledge of available community resources/post-acute care options.
- Collaborates with other departments or disciplines to achieve desired quality outcomes.
- Makes sure quality of core processes in department.
- Applies the principles of safety and infection control to ensure the safety of all patients.
- Demonstrates superior interpersonal and communication skills.
- Works within a team to achieve patient and team goals.
- Shares and initiates regular and professional communication with co-workers.
- Participates in regular staff meetings.
- Identifies opportunities to improve processes based on feedback from staff and customers.
- Provides primary health care and performs selective medical services under the direction of practice physicians.
- Obtains complete history and physical data on patients.
- Reviews patient records.
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
- Establishes presumptive diagnosis.
- Orders appropriate laboratory and diagnostic procedures.
- Performs independent evaluations and treatment procedures in emergency situations.
- Provides monitoring and continuity of care between physician visits.
Qualifications
- Education: Associate's Degree in Nursing required. 4 Year/Bachelor's Degree, BSN, preferred.
- Experience: 2 or more years of healthcare experience preferred. Case Management or related experience preferred.
Additional Skills/Requirements
- Knowledge/Competence of Microsoft Office products and keyboarding.
- Weekend and holiday coverage.

- Proficient communicative, auditory, and visual skills.
- Attention to detail and ability to write legibly.
- Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
Physical Demands
- Constant standing, frequently walking, and bending/stooping.
- Use of safety equipment, such as HEPA mask, for infection prevention: Yes.
- On call responsibilities as directed: Yes.
- Ability to travel between campus buildings, remote facilities, and out of town as needed: Yes.
Pay
The base compensation range for this role is $31.22 - $50.50 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.
