Responsibilities

• Perform Discharge Planning evaluations including risk assessments for defined population within expected time frame
• Provide Assessment and ongoing reassessment of a patient's diagnosis, prognosis, and anticipated care needs and goals
• Collaborate with other disciplines; actively participate in interdisciplinary rounds
• Collaborate with the social worker daily to identify patients in need of social work intervention or with potential post-hospital discharge needs
• Intervene and educate patients and families regarding anticipated complex home care needs including providers which are applicable to the patient’s goals and treatment preferences
• Utilizes teach back methodology
• Acquire patient choice for skilled home care providers and educate about payer in-network providers

Requirements

• BSN preferred or must obtain within three (3) years of hire date
• Licensed as a Registered Nurse in the State of New Jersey
• Minimum of three (3) to five (5) years of experience in acute care nursing
• Good critical thinking and communication skills
• Must be willing to work on some weekend days and some holiday days

Pay

Salary Range: 85,467.00 - 141,066.00 USD

In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials. The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
