Case Manager RN
Description
The Care Transitions Case Manager collaborates with physicians, social services, nurses, community agencies and other members of the care team to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Care Transitions Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition of patients through medically appropriate levels of care.
Qualifications
- Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment
- Case Management Certification preferred
- Five years Clinical RN experience required
- Diverse knowledge of community services, resources and programs
- Previous case management experience preferred
What You'll Do
- Captures and documents patient care progression throughout the continuum
- Collaborates with physicians, nursing and other members of the multidisciplinary team to effect timely, appropriate patient management
- Coordinates with social workers to ensure a warm handoff with caregivers at next site of care
- Identifies, triages, and assists in resolving delays and obstacles to discharge
- Conducts face-to-face assessments and documents primary and back-up discharge plan on all patients
- Serves as a resource person for families, patients, employees and other health care professionals
- Creates processes to anticipate and address specific situations within the department
- Applies quality improvement methods and techniques to improve case management processes
- Maintains required department confidentiality and abides by HIPPA regulations
- Communicates with and performs duties following the philosophy of MercyOne North Iowa Medical Center and the Sisters of Mercy
- Supports and abides by all medical center, departmental, and safety policies and procedures
