Thompson was founded in 1886 as an orphanage and now operates across the Carolinas, Florida, Tennessee, and Kentucky. Thompson's continuum of care includes prevention, mental health services, and foster care, all of which are trauma-informed and evidence-based/evidence-informed.

What will you do as a Case Manager?

As a Case Manager in the Residential Department, your primary purpose is to coordinate care related to clients assigned in their programs, including discharge planning, CFT coordination, communicating with external stakeholders, and managing other aspects of the client’s care. Your performance will be measured by your individual outcomes, achieving your individual targets/goals, your contribution to your overall program team/department, and your buy-in to the culture of the organization. Display individual proficiency and effectiveness in the role. Be punctual, arrive on time, and prepared. Learn and utilize the organization’s systems, processes, and procedures as instructed. Display a high degree of being open to coaching/feedback regarding performance and areas of improvement. Contribute to a positive team culture through your actions, attitude, and communication. Consistently deliver excellent customer service to your clients/customers. Conduct yourself in the highest standards of ethics and integrity while displaying emotional intelligence in your day-to-day work with customers, peers, and leadership. Represent the organization internally and externally through your communication and conduct. Display trauma informed practices and principles in your interactions with customers, with peers, and with leadership. Oversee the coordination of successful discharges, referrals, and general needs of the client on your caseload. Coordinate Child and Family Team meetings, external and internal stakeholder meetings, and drive communication around the client’s needs. Other duties as assigned.

Requirements

  • Must have a valid Drivers' License

  • Meet any credentialing, licensing, and privileging standards as required of the job

  • Bachelor’s degree in human services or related field

  • A minimum of 2 years of relevant professional experience

  • Proficient in Microsoft Office Suite application software

  • Excellent written and oral communication skills

  • Ability to work on-site at the assigned location during scheduled hours

  • Ability to work well with different internal and external stakeholders
