About the role
Coach, evaluate, and develop your case management team, driving a high-performing culture of advocacy and clinical excellence. Oversee daily workflows, streamline case management processes, and champion efficient resource utilization and discharge planning. Serve as the primary escalation point for complex, high-risk patient care tracking, offering expert guidance on challenging case barriers. Collaborate closely with leadership on daily operations, contributing valuable insight to hiring, training, and departmental performance.
Responsibilities

Requirements
- Licensure / Certification / Registration: Required Credential(s): Registered Nurse obtained prior to hire date or job transfer date. Preferred Credential(s): Case Manager credentialed from the American Case Management Association (ACMA). Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
- Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required. Master's degree preferred.
Qualifications
