About the role

We are seeking a dedicated Case Manager to join our Aetna team. As an industry leader in serving dual eligible populations, you will utilize best-in-class operating and clinical models to improve the health and wellness of our members. Your role will involve collaborating with members, providers, and community organizations to address the full continuum of health and social needs.

Responsibilities

  • Assess and analyze members' medical and/or vocational status
  • Develop a plan of care to facilitate appropriate condition management
  • Communicate with members/clients and other stakeholders telephonically or in person
  • Prepare required documentation of case work activities
  • Collaborate with internal multidisciplinary team to enhance health outcomes
  • Conduct evaluations of members' needs and benefit plan eligibility
  • Facilitate integrative functions using clinical tools and information/data
  • Monitor member/client progress toward desired outcomes
  • Develop a proactive course of action to enhance wellness and medical outcomes
  • Advocate for options and services to meet comprehensive health needs

Requirements

  • Active and unrestricted Illinois Registered Nurse (RN) License
  • 3-5 years clinical practical experience
  • Experience in care management, discharge planning, or home health care coordination preferred
  • Associate's or Bachelor's Degree in Nursing
  • Ability to travel up to 50-75% of the time within a designated geographic area

Qualifications

  • Resides in Rock Island, Mercer, Whiteside and surrounding counties
  • Confidence working independently, using tools to collaborate and connect with teams virtually
  • Excellent analytical and problem-solving skills
  • Effective communications, organizational, and interpersonal skills
  • Proficiency with standard corporate software applications, including Microsoft Word, Excel, Outlook, and PowerPoint
  • Efficient and effective computer skills including navigating multiple systems and keyboarding

Pay

The typical pay range for this role is $66,575.00 - $142,576.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors.
