About the role

We're expanding our commitment to health and care, and we need someone like you to join our Community Care team. As a key member of our team, you'll play a vital role in improving the health and wellbeing of our members.

Responsibilities

  • Liaise with members, providers, and community organizations to address comprehensive health needs.
  • Implement and coordinate case management activities for catastrophic cases and chronically ill members.
  • Communicate with members, clients, and stakeholders to ensure timely return to work or optimal functioning.
  • Prepare required documentation and interact with internal multidisciplinary teams to enhance health outcomes.
  • Evaluate members' needs and benefit plan eligibility, facilitating integrative functions using clinical tools.
  • Monitor member/client progress towards desired outcomes through assessment and evaluation.

Requirements

  • Active and unrestricted Compact Registered Nurse (RN) licensure in Alabama, Arkansas, Maryland, Mississippi, North Carolina, South Carolina, Tennessee, Virginia, Georgia, or Florida.
  • 3+ years of clinical practical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, Home Health, and case management.
  • 2+ years of case management, discharge planning, and/or home health care coordination experience.
  • Proficiency with standard corporate software applications, including Microsoft Word, Excel, Outlook, and PowerPoint.
  • Efficient and effective computer skills, including navigating multiple systems and keyboarding.

Qualifications

  • Bachelor's Degree in Nursing preferred.
  • Associate's Degree in Nursing required.
  • Active and unrestricted Compact Registered Nurse (RN) licensure in Alabama, Arkansas, Maryland, Mississippi, North Carolina, South Carolina, Tennessee, Virginia, Georgia, or Florida.
  • Experience with standard corporate software applications, including Microsoft Word, Excel, Outlook, and PowerPoint.
  • Effective communications, organizational, and interpersonal skills.
  • Certified Case Manager (CRC, CDMS, CRRN, COHN, or CCM) preferred.

Skills

Strong communication, problem-solving, and time management skills are essential. Proficiency with case management processes and regulatory requirements is also crucial.
