Job Overview

Location/Schedule: This position is co-located between Rain Garden and Alberta Plaza, supportive housing sites located in Wilsonville and NE Portland, OR, respectively. The schedule for this position is Monday through Friday, 8:30 a.m. to 5:00 p.m.

Position Description

The Case Manager, QMHA works as a valuable part of the treatment team for clients in a community health center setting. The Case Manager is responsible for assisting clients access a variety of services and supports. Additionally, this role serves as an advocate and works with clients to meet their goals and needs. This position must understand, appreciate, and respect the diversity and cultural differences within our Cascadia community. As such, it is expected that this position promotes integrated care, our vision of trauma-informed and person-first approaches and helps create a work environment of inclusion, safety, and acceptance.

Essential Responsibilities

  • Provide mental health case management and skills training with clients as indicated in their treatment plan, and as directed by the treatment team.
  • Work with the treatment team on assessing treatment needs, support development and monitoring, provide consultation to mental health aides, and transition planning -including completion of referrals, attending screenings, advocacy, maintaining housing, obtaining resources, benefits and consultation regarding client needs.
  • Provide individual and group: skills training, CPST, activity therapy, and meet productivity expectation.
  • Effectively engage clients to participate in treatment services.
  • Integrate best practices into the day-to-day operations including: Person-Centered Approach, Harm Reduction, Trauma Informed Care, Motivational Interviewing, Strength-Based Approach and Recovery Model.
  • Create and maintain cooperative relationships with outside providers in securing benefits, advocating for clients, and connecting to acute care providers.
  • Career Development and Supervision to support clients in developing and maintaining structured activities in alignment with their personal goals and any required conditions for community integration.
  • Other duties as assigned.

Regulatory & Compliance

  • Comply with laws, regulations, policies, and procedures under which the program and company operates.
  • Aid in crisis situations as appropriate following protocols and procedures in coordination with the treatment team.
  • Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.
  • Maintain accurate and up-to-date documentation that ensures compliance with all agency policies and procedures and local, state, and federal regulations, and billing requirements.
  • Understand safety regulations and evacuation procedures; participate in regular safety drills in compliance with program policies and procedures.
  • Complete annual employee training in compliance with program and company requirements.
  • Provide guidance, support, and training to other staff in alignment with company mission and values.
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and company meetings.

Qualifications

  • Education: Required: Bachelor’s degree in a behavioral science field from an accredited college or university or three years of fulltime professional experience providing direct service to individuals in a social service setting. Demonstrate the competencies to become credentialed as a Qualified Mental Health Associate (QMHA), as outlined in the Oregon Administrative Rulings (OARs). Certification must be kept current and in compliance with OARs through MHACBO.
  • Experience Required: A minimum of 1 year experience working with adults with severe and persistent mental illness.
  • Specialized Knowledge, Skills, And Abilities: Effective communication and active listening skills. Effective interpersonal and customer service skills. Ability to work proactively and effectively in a team and independently. Knowledge of Microsoft Office suite. Operate computer hardware and software at a level needed to effectively perform job functions.

Other

  • Driving is required, an employee must possess a valid driver’s license, a minimum of one year driving history, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
  • All client-facing positions must have the ability to perform Basic Life Support (BLS) and properly use AED equipment. BLS certification training will be provided within the first 60 days of hire. Certification must remain current.

Working Conditions

  • Environmental: Work is performed in one or more of the following: clinical office, acute care settings and community setting including client residences, correctional facilities, shelters, hospitals, public places and other social service sites. Meal periods and breaks are subject to interruption. Much of the work is performed during regular business hours with the potential of extending into afterhours only for crisis response.
  • Mental: The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach. In addition, this position requires exercising sound judgment. Decision-making may occur in a moderate to high stress situation where a client may be experiencing a crisis.
