Case Manager - PRN/Per Diem

OhioHealth Rehabilitation Hospital, a joint venture with Select Medical, provides exceptional inpatient rehabilitation for patients with brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas.

About the role

The Case Manager coordinates health care decisions using a systematic approach to ensure treatment plans that improve quality and outcomes, coordinate care across the continuum, promote cost-effective care within allotted time frames, assure payment of hospital-based services meeting utilization management criteria, and implement safe, appropriate discharge plans. The role also assesses psychosocial needs and provides intervention as part of discharge planning.

Requirements

Minimum Qualifications

• Current licensure per state guidelines in a clinical or related discipline OR a Bachelor's or Master's degree in a health or human services discipline

Preferred Qualifications

• Previous experience in Case Management and Discharge Planning
• CCM Certification
