About the Role

The Case Manager provides comprehensive case management to victims of domestic violence using a trauma-informed care, strengths-based approach to support quality of life and community functioning.

Responsibilities

  • Develops effective, trusting relationships with program participants utilizing a trauma-informed and strength-based approach.
  • Models positive interactions, verbal guidance, and effective communication.
  • Utilizes the Housing First Model to advocate for and facilitate participants’ access to community resources, housing, school programs, vocational opportunities, recreational activities, and mental health services.
  • Assists participants in developing natural supports and connecting with social support networks.
  • Assists participants with benefits acquisition and maintenance.
  • Develops and maintains outreach and working relationships with landlords, property management teams, and community partners to create housing plans.
  • Evaluates participants’ individual needs and completes timely service plans with short- and long-term goals; provides follow-up and evaluation.
  • Coordinates and monitors services, including tracking participant activities in relation to the service plan.
  • Documents weekly and emergency case notes, submits resident files, completes reports, and enters documentation into the HMIS system.
  • Identifies and provides emergency crisis services; conducts clinical assessments and coordinates additional services as needed.
  • Maintains ongoing communication with a multidisciplinary team to assist participants in obtaining and maintaining housing and accessing services.
  • Provides life skills training on a one-on-one or group basis, including budgeting, problem-solving, social skills, parenting, and behavior modification.
  • Assists residents with budgeting and monitoring payment of necessary bills.
  • Performs other duties as assigned.
  • Demonstrates exceptional customer service in all interactions, prioritizing the needs of participants to support the mission of empowering people.

Requirements

  • Bachelor's Degree in Social Work, Sociology, Psychology, or related field.
  • 1+ years of experience in a social service agency and/or 1+ years in a customer service-focused environment.
  • Experience with mental health and/or victims of domestic violence populations is a plus.
  • Must maintain regular and acceptable attendance as determined by the employer.
  • Valid Driver’s License with a clear record.
  • Must have daily access to a vehicle without prior notice.
