About the role
The ODR Case Manager works with clients recently released from jail, who are moving from homelessness to in permanent supportive housing. Clients face complex trauma, medical, mental health, and substance use barriers.
Responsibilities
- Work with a diverse caseload of 15 clients, providing intensive case management.
- Maintain weekly contact with all clients and document contacts in County-approved database.
- Ensure case management services reflect the "Whatever It Takes" model, emphasizing customer choice, harm reduction, trauma-informed care, and psycho-social rehabilitation.
- Link clients to appropriate medical, mental health, substance abuse, and vocational resources.
- Absorb clients into the program until they achieve stable housing.
- Develop and maintain a complete, accurate, and current client file with all required documents and data.
- Transport clients to various agencies and healthcare providers in the greater Los Angeles area.
- Facilitate daily activities, schedule appointments, budget, socialize, and maintain recovery for clients.
- Collaborate with other clients and staff teams, participate in weekly meetings and trainings.
Qualifications
- High School Diploma or GED, or equivalent.
- Bachelor’s degree preferred; minimum two years’ case management experience working with individuals experiencing homelessness, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience.
- Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
- Pass a background check for jail clearance.
- Demonstrated knowledge of case management.
- Knowledgeable in a high tolerance Harm Reduction model with clients who have multiple barriers.
- Strong computer skills, including basic proficiency in Microsoft Word, Outlook, and Excel programs.
- Detail-oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills.
- Able to obtain and maintain CPR/First Aid certification.
Preferred Qualifications
- Bachelor’s degree preferred.
- Minimum 30 WPM typing speed preferred.
Work Environment
Combination of field and office environment. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs. On occasion walk or drive to different local sites. Will necessitate working in busy and loud environments. Exposed to elements like cold, heat, dust, noise, and odor. May need to bend, stoop, twist, and sit throughout the day.
