About the role
The Case Manager RN supports patients and families throughout their care journey by coordinating services, developing personalized care plans, and facilitating safe transitions.
Responsibilities
- Conduct comprehensive assessments
- Identify at-risk patients
- Collaborate with healthcare teams and social services
- Implement and document care plans
- Monitor progress and evaluate outcomes to ensure quality care and effective resource utilization
Qualifications
- Minimum of 2+ years in case management or 3+ years in a clinical setting
- Associates Degree in Nursing
- BLS certification required
- Current RN licensure
- Preferred InterQual experience
- Master's Degree and additional certifications are advantageous
Skills
- Strong communication, assessment, collaboration, and documentation skills
- Ability to coordinate multidisciplinary efforts and advocate for patient needs
