Navigate provides intensive, community-based services to individuals with severe mental illness. These individuals often have complex mental health needs, including acute psychosis, mood dysregulation, histories of complex trauma, and co-occurring substance use disorders.

Position Specific Duties & Responsibilities

  • Work with clients to identify their personal preferences regarding work and school goals, including career interests, and provide the necessary supports to help the person achieve those goals.
  • Attend and participate in daily organizational meetings and weekly treatment team meetings with a multi-disciplinary team, utilizing a team approach in working with Navigate clients.
  • In the initial assessment phase, complete an Education and Career Inventory with the clients to develop a shared understanding of the individual, their employment or school history, and their preferences with respect to school, types of work, and career backgrounds.
  • The job search or school enrollment phase is aimed at providing practical assistance to the client in finding the type of job or school program that they are interested in participating in.
  • The final phase involves providing follow-along supports that are aimed at giving clients practical assistance to facilitate good performance, prevent crises from arising, and promote success at work or school.
  • Be available to respond to crises in the community, utilizing agency protocols for crisis diversion. May include completion of Third-Party Affidavits and coordination with law enforcement to ensure individual is safe.
  • Utilize agency issued cell phone during work hours. Staff are expected to be responsive to cell calls and agency approved electronic communication, when scheduled to work.
  • Demonstrate comfort and flexibility while working with clients in difficult life circumstances, which may include individuals who are homeless or living in substandard conditions.

Qualifications

  • Must have a Bachelor's degree in Social Work, Psychology, or a related social services major, which qualifies for Case Management II certification with the Oklahoma Department of Mental Health.
  • Candidates must have CM-II certification or obtain CM-II certification within 60 days of employment.
  • Candidates with a previous background working in community mental health, inpatient psychiatric facilities, criminal justice, home health, or similar settings are preferred.
  • Strong candidates with a passion for helping individuals with severe mental illness will also be considered.
  • Must have a valid driver’s license and satisfactory driving record; use of personal automobile will be required for travel to locations outside of primary office.
