About the Role
The Case Manager I provides a comprehensive array of support focused on skill building and case management activities to help clients achieve a meaningful level of independence and well-being. This role involves completing screenings and assessments, developing person-centered care plans, and delivering services through a team approach. Responsibilities include rehabilitative supports, skill building for activities of daily living, resource access, psycho-education groups, socialization, and community integration.
Responsibilities
- Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
- Enhance clients’ daily living skills through assessment and teaching modalities (e.g., modeling, prompting, hands-on assistance).
- Develop, implement, and monitor person-centered plans with clients throughout the intake and discharge processes.
- Coordinate and assist clients with case management needs, including referrals, entitlements, money management, treatment access, medical care, transportation, housing, wellness/nutrition, legal services, education, and employment.
- Identify potential barriers to clients’ goals and link them to appropriate services.
- Meet regularly with clients to review progress, mental health stability, appointments, and activities.
- Support clients in learning local transportation and provide transportation as needed (may include using personal vehicles).
- Conduct regular apartment/room inspections and assist clients in maintaining clean living spaces, including hands-on assistance.
- Develop and maintain relationships with families, natural supports, and external providers; advocate for clients as necessary.
- Facilitate or co-facilitate psychoeducational groups.
- Complete service entry documentation per contract and agency requirements.
- Administer breathalyzer tests and urinalysis samples as applicable.
- Conduct program rounds as indicated.
- Attend and participate in staff meetings, AllTreater’s meetings, rounds, and supervision.
- Perform other duties as assigned.
Requirements
- Bachelor’s degree in a related field and one (1) year of relevant experience; OR a high school diploma or equivalent and three (3) years of human services experience, including at least one (1) year of service with Continuum.
- Primarily works within residential 12-hour and 24-hour programs.
- Provides direct support, skills training, and daily structure to clients under regular supervision.
- Manages caseload within a structured program environment with ongoing supervision, mentorship, and team collaboration.
- Implements established treatment and service plans developed in collaboration with supervisors and clinical staff.
- Performs care coordination, documentation, crisis response, and service planning with regular supervisory support.
- Develops foundational competency in client engagement, documentation, and community resource coordination.
Schedule
Full-time, 40 hours per week. Sunday through Thursday, 1st shift.
