Case Manager - HopePath for Families (in-CIRCLE) - Meridian, MS

Job Responsibilities

• Coordinate with local MCPS staff and contact the family within 24 hours after the referral is accepted.
• Aid eligible individuals in accessing medical, social, educational, vocational, and other services.
• Ensure case management services are provided and documented according to programmatic requirements.
• Maintain and document progress toward case plan goals and interventions.
• Facilitate initial family team meetings with MDHS and update the Crisis Management Plan as needed.
• Conduct weekly ISP reviews and document progress towards service goals.
• Assess families' strengths and needs and utilize community resources to address their needs.
• Act as a positive role model in all interactions with participants.
• Provide direct case intervention services in various settings including home, school, office, and community.
• Coordinate and provide social services that are family-driven and community-based.
• Drive families using personal vehicle for organization business and complete satisfaction surveys as assigned.
• Prepare and complete program reports as assigned.

Required Qualifications

• Bachelor’s degree in Social Work or a related field, with at least two (2) years of experience working with children and families.
• Demonstrated competence in working with children and families in the child welfare system.
• Competence in completing documentation and reports related to services provided.
• Computer skills required.
• At least 21 years of age, valid driver license, good driving record, legal auto liability insurance, and willingness to use personal vehicle for organization business.
