Program And Position Summary
The Salvation Army of the Twin Cities is dedicated to "Doing the Most Good" by providing holistic, person-centered services that empower individuals and families. Programs include case management, food, clothing, utility and rent assistance, housing and shelter programs, and connections to community partners. Among these, Harvest Hills and HOPE Harbor are supportive housing programs that help individuals transition from crisis to stability through safe housing and comprehensive case management. Participants receive assistance with employment, education, life skills, and access to community and spiritual supports.
About the Role
This position plays a key role in promoting resident self-sufficiency, stability, and successful program completion. The position requires a Bachelor’s degree in a relevant field, with state board licensure required if the degree is in social work. Previous social services experience is required, with long-term case management experience preferred.
Essential Functions
- Case Management (50%): Provide long-term strength-based, person-centered, holistic case management. Develop and implement service plans, monitor progress, and support participants in achieving their goals.
- Participant Information Management (25%): Maintain accurate and timely records in both physical and electronic files. Ensure compliance with documentation standards for case notes, assessments, and updates.
- Community and Organizational Engagement (5%): Participate in internal and external initiatives to promote the Harvest Hills program and expand participant resource networks.
- Participant Recruitment and Program Promotion (5%): Conduct outreach, build referrals, and engage with the community to recruit eligible participants and raise awareness of program services.
- Education Facilitation (5%): Facilitate individual and group sessions on topics such as life skills, financial literacy, and personal development, tailoring content to participant needs.
- Property Management Collaboration (5%): Work collaboratively with property management to support successful resident tenancy.
- Seasonal and Event Collaboration (5%): Support seasonal and special events to align with program goals and community needs.
Requirements
- Must successfully pass background checks, possess a valid driver’s license, and meet MVR clearance standards.
- Strong interpersonal and communication skills.
- Excellent organizational skills with attention to detail.
- Maintain confidentiality with integrity and professionalism.
- Demonstrate honesty, diligence, and a strong work ethic.
- Proficient in Microsoft Office, databases, and general PC use.

Wage
$21.03-$24.49 per hour, 40 hours per week

Working Conditions
Primarily sedentary work, involving extended periods of computer use. Occasional lifting up to 20 pounds. On-site position, not eligible for a hybrid, remote-friendly work arrangement. Requires effective communication with participants, colleagues, and community partners.
